The Atomic Weapons Establishment (AWE) has published an independent report into a fire which broke out in a building containing explosives at AWE Aldermaston in August 2011.
The report highlights a string of safety failings, including the holding of explosives in the building contratry to safety regulations and a failure to follow proper procedures by stressed staff - who planned to work a 16 hour overnight shift.
